Forms for Families
Enrollment forms will be sent electronically via Brightwheel once you have enrolled and the deposit is made. Families will also need to provide their child's most recent physical, updated immunizations, and school clearance form from their child's medical provider. Please note, the enrollment forms and all medical paperwork are required by the state of New Hampshire's DHHS and must be updated annually.

The medication form is only required if your child will need medication administered while in attendance. Prescribed medication must be in the original packaging, with the prescription label. Medication that is not prescribed can be administered only when accompanied by a doctor's note.
Please note, all students must be immunized to attend.
